A Brief History of Skincare-MD
By Peter T. Pacik, M.D., F.A.C.S.

Once upon a time, a physician-based advanced skin care program consisted of a variety of peels, usually done in the operating room under anesthesia. By removing the outer layers of skin, healing took place from the deeper dermal elements, thereby creating "new" skin. We called this "resurfacing." Thirty years ago, during my general surgery and plastic surgery residencies, the deep phenol peel was common. We could take any patient with deep wrinkling and create a smooth face. The problem was the bleaching after effect. So now we had smooth skin on a severely bleached face! (Thank goodness for makeup!)

Lighter peels were developed; however, our moisturizing products were mostly primitive. The first major breakthrough for exfoliation was Retin A. This came in a variety of strengths and patients were "graduated" to stronger doses under the care of a physician. The products were helpful but caused considerable drying, redness and sun sensitivity in some of our patients.

As I continued in my own practice, there was a slow movement from reconstructive procedures to more cosmetic surgery. Almost any patient requesting a face lift is also a candidate for better skin care, and so the two concepts worked together hand in hand. We worked with a number of skincare product manufacturers and tried many products as well as many regimens. Some were complicated "7-step" programs, and were used together with facial shaving techniques to get better penetration of the exfoliants. The shaving techniques gave way to our modern microdermabrasion. Better light office peels were developed and the products continued to improve in terms of exfoliation, moisturization and protection.

Nowadays, full strength phenol peels are rarely done, and the emphasis is on lighter resurfacing techniques (laser, dermabrasion and lighter peels) combined with quality advanced skincare products to help patients achieve more lovely skin. MD Forte, our advanced skin care program, smoothes and rejuvenates tired, aging, or sun-damaged skin. The MD Forte product line continues to expand to include more effective facial, hand and body therapy. Facial Cream III and Skin Rejuvenation Lotion III, both with 30% glycolic compound, are the latest products designed to help diminish the signs of skin aging. For patients who have progressed and are comfortable with the 20% strength of glycolic compound, the higher concentration is even more effective for exfoliation; so much so, that microdermabrasion and peels may be spread further apart. Replenish Hydrating Cream, glycolic free, helps restore the skin's natural moisture balance so it feels softer and silkier, and looks healthier. The Hand and Body Cream, with 20% glycolic compound, is an excellent hydrating cream for the entire body. Used once or twice a day, it helps to eliminate rough, callous areas on heels and feet. The advanced skin care program has a long history of effectiveness and we are thrilled to have so many satisfied patients and customers.
